Book the rooftop ticket with elevator access — the stairs are punishing. Walk the marble terraces at the upper levels where the scale of the Gothic engineering becomes visceral. The interior is vast and cold in a way that medieval cathedrals should be. The crypt with San Carlo Borromeo's remains is downstairs. Spend an hour minimum.
Early morning for rooftop access before crowds (8am opening). Late afternoon for golden light on the marble facades. Avoid midday in summer when the white marble becomes blinding. Winter mornings are atmospheric with fog.
Construction began in 1386 and the final details were not completed until 1965. The Madonnina statue at the peak (108 metres) is Milan's patron symbol — no building in the city was allowed to exceed its height until the 1960s. Book tickets online days in advance to skip the queue. Dress code enforced (shoulders and knees covered). The archaeological area beneath the Duomo includes 4th-century baptistery ruins.
